Intake Appointment — New Patient Evaluation

$350 self-pay · all-inclusive

  • A comprehensive evaluation: your history, your health, and your goals
  • Review of past medications — what helped, what didn’t, and why
  • Labs or GeneSight testing ordered when they will change the plan
  • Clear direction by the end of the visit, decided together

30 minutes

Follow-Up Appointment

$250 self-pay · all-inclusive

  • A real review of how the medication is working in your actual life
  • Careful adjustments — or deprescribing when it serves you
  • Brief focused therapy included in every visit
  • Whole-person attention: sleep, nutrition, movement, physical health

How Booking Works

    Sign the treatment intention agreement and share your preferred days and times.

    Complete Your Forms

    Complete the intake forms and consents online — about 10 minutes.

    Pick Your Slot

    Choose an open appointment slot and you’re booked. Your insurance is verified before the first visit, so your exact copay is known up front.

Fees & Insurance

Self-pay: $350 for the 60-minute new patient evaluation and $250 for 30-minute follow-ups — all-inclusive, with no facility fees and no surprise balance bills.

Insurance: in network with Aetna, Cigna/Evernorth, UnitedHealthcare and Optum, the Blue Cross Blue Shield family, Oscar, Oxford, Carelon, Providence, and more. With insurance, the typical copay is $0–$40 for the majority of plans, and every plan is verified before your first visit so you know your exact copay up front.

Also accepting Medicare in AZ, CA, CO, TX, and UT, and Medicaid in OR and WY.

A Note on Controlled Substances

If you are prescribed a controlled substance, most states require one in-person visit each year. We’ll plan that together, well in advance.
