Thoughtful Starts
New medications are started for a clear reason, at a sensible dose, with a plan for how we’ll judge whether they’re working.
Medication management is the whole practice at PsychMed Care — not an afterthought, and never a five-minute refill. Thoughtful starts, careful changes, and deprescribing when it serves you.
Good medication management is a process, not a prescription pad. It starts with a 60-minute comprehensive evaluation and continues in 30-minute follow-ups where the plan is reviewed against your actual life — what improved, what didn’t, what it is costing you in side effects, and what to do next.

Adjustments are made one at a time and followed closely, so we know what helped — and what didn’t.
When a medication no longer serves you, coming off it safely is part of the job. Less medication, when less is right, is a success — not a failure.
“A prescription at the first visit happens sometimes, when it’s clearly the right call. Often I’d rather use the first appointment to understand the full picture before starting or changing anything. We decide together.”Karel Winner, DNP, PMHNP-BC
Your intake is a full 60 minutes precisely so that first decisions are good ones. If something is clearly indicated, we start it. If the picture needs labs, records, or a GeneSight result first, we say so — and you still leave with clear direction.
Follow-up appointments are 30 minutes — every time. That is long enough to review how the medication is really working, make careful changes, and include brief solution-focused therapy and whole-person attention in the same visit.
No five-minute refills. If a medication is worth taking, it is worth reviewing properly.
For older adolescents (17 and up), adults, and seniors — in an LGBTQIA+ affirming practice.
If you are prescribed a controlled substance, most states require one in-person visit each year. We’ll plan that together, well in advance — no surprises.
